Coordinated Veterans' Care
Coordinated Veterans’ Care (CVC) is a new program that aims to improve the wellbeing of veterans, war widows/ers and their dependents and to keep them out of hospital.
This Department of Veterans’ Affairs program offers funded planned and coordinated care, led by a general practitioner (GP) with a nurse coordinator, to Gold Card holders who have chronic conditions and complex care needs and are at risk of being hospitalised.
GPs who decide to be involved in the CVC Program are required to:
- prepare for the program
- enrol participants in the program
- provide ongoing care.
Patients can self-refer or be identified by their doctor or the DVA.

Training
Online education
Accredited online learning modules are accessible at www.cvcprogram.net.au/. This training incorporates latest clinical evidence, will further develop your understanding of the benefits and processes of multi-disciplinary care in a primary care environment and will ensure your practice is ready to participate in the CVC program.

Workshops
