Community and Hospital Collaboratives
The Metro North Brisbane Medicare Local will build on the strong relationships already founded with our local public health services. Partnerships with local public hospitals and community health services will aim to improve our community's access to health care.
Collaboratives
Collaboratives of Queensland Health and private hospital executive and staff, Queensland Health primary health care services, local GPs and Medicare Local staff meet regularly to progress initiatives that improve communication between hospitals and primary care services including referral, discharge and clinical pathways and, to raise awareness of service availability.
Through our Collaboratives initiatives in areas including e-health (electronic referrals and discharge summaries), education for health professionals, mental health, refugee health and child health aim to address communication barriers and identify community need.
Collaborative groups current exist with:
Hospitals
- Royal Brisbane and Women’s Hospital
- RBWH Women’s and Newborns Service
- Royal Children’s Hospital
- The Prince Charles Hospital
- UnitingCare Health (including The Wesley Hospital & St Andrew’s War Memorial Hospital)
Primary and community
- Northside Mental Health Service (Servicing TPCH, Redcliffe, Caboolture and Kilcoy)
- Inner North Brisbane Mental Health Service (Royal Brisbane and Women’s Hospital)
- Community Child Health Service (Queensland Health)
- Primary and Community Health Services (Metro North Health Service District)
